Clinical Labs of Hawaii provides forms for patients looking to schedule a COVID-19 test. These forms include locations for test sites and instructions on how to schedule a COVID-19 test. A virus symbol next to a location indicates it as a site that tests for COVID-19.

Drive-thru testing is still available for now at Kapiolani Medical Center and Straub for COVID-19 pre-op testing. They are no longer taking tests on a first-come, first-serve basis, so please make an appointment for testing. We do not know how long these centers will continue to do drive-thru testing as the pandemic continues.
If a patient is scheduled for surgery with Dr. Tokunaga:
For Monday surgery, schedule a COVID test the Thursday before to ensure clearance for surgery. For Friday surgeries, please schedule a COVID test for the Tuesday before surgery.
If a patient is scheduled for surgery with Dr. Yuen:
For a Tuesday surgery, please take a COVID test the Friday before. For Friday surgeries, please schedule a COVID test for the Tuesday before surgery.
